Here's the Skinny:

This is a four-part form. Fill in the blanks. Feel free to type or cut & paste. Spaces marked with an asterisk are required spaces. You won't be able to submit this form until they're all filled.

You will need the following information handy:

  • Biographical information
  • Your resume
  • Reference/Evaluation Letters
  • Quotes from others
  • Audience Analyses
  • Financial Requirements

The completed application will take between 30 - 90 minutes, depending upon your preparation.

Carefully review your entries. You will not be able to return to a page once it has been submitted. This is for security protection.

Once your submission has been reviewed, you will be contacted. If your application is accepted, you will be asked to provide supplemental materials (photographs, samples, etc.)
