Here's the Skinny:

This is a four-part form. Fill in the blanks. Feel free to type or cut & paste.
Spaces marked with an asterisk are required spaces. You won't be able to submit this form until they're all filled.

You will need the following information handy:

  • Biographical information
  • Your resume
  • Reference/Evaluation Letters
  • Quotes from others
  • Audience Analyses
  • Financial Requirements

The completed application will take between 30 - 90 minutes, depending upon your preparation.
